• Limit of $20 off each prescription of VERAMYST for up to 6 total refills of VERAMYST per patient within a calendar year.
• To be eligible for the offer, you certify that: (i) you have read the enclosed program rules and regulations, terms, and conditions, (ii) you have not submitted and will not submit a claim for reimbursement under any federal, state, or other governmental programs for this prescription, (iii) if you are Medicare eligible, you are not enrolled in an employer-sponsored health plan for retirees or a Medicare Part D Plan, and (iv) you will otherwise comply with the terms on the coupon.
